    This form must be completed by Undergraduate (First Year or Transfer) candidates who are:
    • submitting an Art Portfolio through Slideroom for consideration for the B.A. or B.F.A degree programs in the Division of Art 
    • submitting a Film Portfolio through Slideroom for consideration for the B.F.A. degree program in the Division of Film and Media Arts
    This form must be submitted at the same time that you submit your portfolio in SlideRoom: DO NOT submit this form without submitting your portfolio in SlideRoom.
